Virginia MED 40: Bioptic Telescopic Lens Vision Report

Virginia drivers who want to be licensed with a bioptic telescope need their eye doctor to report their vision on this form. It captures changes in vision over the past year, uncorrected acuity, acuity through the carrier lens and through the telescope, horizontal field in degrees, and the telescope's make, power and date received. DMV Medical Review Services reviews it with the MED 41 certification before allowing testing.

Who completes it

The applicant's identifying details go in Applicant Information. An ophthalmologist or optometrist completes the vision history, examination results, telescopic lens, diagnosis and prognosis fields and signs the certification.

Submitting and payment

How to submit it
The eye doctor mails the completed form to Virginia DMV Medical Review Services. Virginia's bioptic information sheet (MED 44) asks applicants to submit MED 40, MED 41 and the license application at the same time, by mail or at a customer service center.

Good to know

  • Visual field testing for this form requires a test object of at least 3 mm, with horizontal degrees recorded for the right eye, left eye and both eyes.
  • The doctor states whether the condition is stable, notes conditions that may require vision re-examination, and recommends how often re-examination should occur.
  • Bioptic licenses carry minimum restrictions: wear the bioptic and carrier lens while driving, daylight hours only, and no bus, truck, commercial or motorcycle endorsements.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

What are Virginia's vision requirements for bioptic drivers?

Virginia's MED 44 information sheet sets 20/200 or better in one or both eyes through the carrier lens and 20/70 or better through the bioptic telescope, which must be mounted to the carrier lens. Horizontal vision must be 70 degrees or better, or 40 degrees temporal and 30 degrees nasal if only one eye is used.

Can a Virginia bioptic driver ever drive at night?

Bioptic licenses start with daylight-only driving, from half an hour after sunrise to half an hour before sunset. According to MED 44, the restriction may be lifted after one year if acuity is 20/40 or better in one or both eyes without field expanders and the licensee passes a night driving road skills examination.
