Virginia DMV MED 20 Sun-Shading Medical Authorization

Virginia drivers whose medical condition requires additional window sun-shading apply to DMV with this form, which also adds vehicles to an existing authorization. A physician, nurse practitioner, ophthalmologist, optometrist or physician assistant certifies that sun-shading is necessary for the patient's health and describes the condition. Once processed, DMV issues a registration card with a sun-shading notation.

Who completes it

The vehicle owner completes the owner, vehicle and certification sections; the medical provider completes and signs the Medical Provider Certification, which is needed for new applications only.

Submitting and payment

How to submit it
Return it to any DMV customer service center, or send it by mail or fax.

Good to know

  • With medical authorization, side and rear windows may be shaded to 35% total light transmittance, compared with 50% for front side windows without it.
  • The law does not allow darker tint than the table permits even with a provider's recommendation, and the provider acknowledges that recommending darker tint exposes the vehicle and owner to a Code violation.
  • Each vehicle must have both left and right outside mirrors to be eligible under Va. Code 46.2-1052 and 46.2-1053.
  • The authorization lasts until the vehicle is sold or transferred or the condition no longer exists, when the shading must be removed. MED 21 certifies removal.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify Virginia's sun-shading medical authorization?

A physician, nurse practitioner, ophthalmologist, optometrist or physician assistant, who checks the matching box, gives license number and contact details, answers whether sun-shading is necessary for the patient's health based on examination, describes the condition and signs under penalty of perjury.

Does a provider need to sign MED 20 again to add a vehicle?

No. The Medical Provider Certification is required for new applications only. A subsequent application, which adds vehicles to an existing sun-shading medical authorization, does not need a new certification from the provider.

How long does a Virginia sun-shading medical authorization last?

Until the vehicle is sold or transferred to another person or the medical condition no longer exists, whichever comes first. At that point the sun-shading must be removed from the vehicle.
