MA RMV MAB118: Psychiatric Evaluation Form

When the Massachusetts RMV receives information that a driver's mental health condition may affect driving, it asks for MAB118. The clinician gives the psychiatric diagnosis using DSM-5 or ICD-10, describes the extent, frequency and control of symptoms, such as orientation or dissociative episodes, any loss of consciousness, and medications, then certifies whether the patient is medically qualified to drive.

Who completes it

A Massachusetts-licensed medical doctor or a psychiatric nurse practitioner completes the whole form after the patient signs the release, certifying under the pains and penalties of perjury with an NPI, or Board of Registration number if there is no NPI.

Submitting and payment

How to submit it
Mail or fax it to the RMV's Driver Control Unit, as printed on the form. The current version is MAB118 09/25.

Good to know

  • Three outcomes are offered: qualified, not qualified, or unable to determine with a competency road examination recommended. Unlike MAB106, there is no adaptive equipment option.
  • List all medications with dosages and state whether, alone or in combination, they are likely to interfere with safe driving.
  • If a police report is attached, confirm whether you have read it and are aware of the reported incident involving the patient.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete the Massachusetts RMV psychiatric evaluation?

A medical doctor licensed to practice in Massachusetts or a psychiatric nurse practitioner. The patient first signs the release authorizing the clinician to discuss and share relevant records with the RMV, and the clinician certifies the answers under the pains and penalties of perjury.

How should the diagnosis be written on MAB118?

Item 1 asks for the psychiatric condition using a DSM-5 or ICD-10 diagnosis. Item 2 then asks for the extent, frequency and control of symptoms that could affect driving, with examples such as whether the patient is oriented in all spheres or has dissociative episodes.

What if the patient's condition involves a seizure or loss of consciousness?

Item 4 asks for the type and date of the last episode. Massachusetts treats seizure, syncope and other altered consciousness under a six-month episode-free policy, evaluated on the separate Loss of Consciousness Evaluation Form, MAB108.
