NY DS-874: Article 19-A Bus Driver Medical Exam

Bus carriers in New York subject to Article 19-A, including school bus operators, need this two-page exam report for their drivers. The driver supplies a health history, then the medical examiner records vision, blood pressure, hearing, urinalysis and a 12-system physical exam and certifies whether the driver is qualified under Commissioner's Regulation 6.10. The carrier keeps the original in the driver's 19-A file.

Who completes it

The driver or carrier completes Section 1 and the driver completes the Section 2 health history; the medical examiner reviews it, completes Sections 3 to 8 and signs. An advanced practice nurse who is not an NP needs a supervising physician's certification.

Submitting and payment

How to submit it
The report stays with the employer: the carrier must keep the original, not a photocopy, in the employee's 19-A file, and any photocopy must identify where the original is kept.

Good to know

  • Vision standard: at least 20/40 in each eye with or without correction, at least 70 degrees peripheral in the horizontal meridian in each eye, and recognition of red, green and amber signals.
  • Record blood pressure numerically from at least two readings. Readings consistently above 160/90 may need further testing before the driver is qualified to operate a bus.
  • Urinalysis is required. Protein, blood or sugar in the urine may indicate further testing to rule out an underlying medical problem.
  • For follow-up exams, complete only the items needing follow-up from the prior exam. Use continuation sheet DS-874C when more space is needed.

Frequently asked questions

Where are the standards for the DS-874 exam?

Section 6.10 of the Commissioner's Regulations (15 NYCRR 6) holds the complete standards and instructions. The form says they are posted on the DMV website's Article 19-A pages and are also available from the driver's carrier or from the DMV Bus Driver Unit.

Can a nurse practitioner or PA sign the DS-874?

Yes. The examiner checks a title: examining physician, nurse practitioner, physician assistant or advanced practice nurse. If an advanced practice nurse who is not an NP performs the exam, the supervising physician must certify that the examiner acted under their direction and supervision.

What restrictions can the examiner place on a 19-A driver?

Section 8 lets the examiner find the driver qualified, not qualified with a reason, or qualified with restrictions: only with corrective lenses, only with a hearing aid, only with prosthetic devices or equipment modifications, certification every six months for a diabetic condition, or another stated restriction.
