NY DMV DS-6 Physician's Reporting Form

New York clinicians use this one-page form to tell the DMV Medical Review Unit about a driver whose physical or mental impairment may affect safe driving. It identifies the driver, describes the condition and medications, and records the clinician's opinion: either the patient should be evaluated by DMV, or the condition prevents safe driving and privileges should be suspended.

Who completes it

A licensed physician, physician assistant or nurse practitioner completes all three parts (driver identification, description of the condition, and the practitioner's own identification and certification), then signs and dates the form.

Submitting and payment

How to submit it
Email the completed form to the DMV Medical Review Unit or mail it to the unit in Albany; the form's instructions give both routes.

Good to know

  • Part 1 marks only last name, first name and city as required; if the date of birth is unknown, an approximate age is accepted.
  • The reviewer may ask for more medical detail or request information from a pertinent sub-specialist, such as a cardiologist or neurologist.
  • After a physician report, DMV may suspend the license until a physician certifies that the condition is controlled.
  • Non-physicians, such as family members, report on DS-7, Request for Driver Review. DMV does not reveal a reporter's identity in response to a FOIL request.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is physician reporting to the New York DMV mandatory?

No. New York has no mandatory physician reporting, so DS-6 is a voluntary report. Once a report arrives, DMV may suspend the license until a physician certifies the condition is controlled, and DMV does not reveal the identity of a person who reports a driver's medical condition in response to a FOIL request.

Who can sign the NY DS-6?

It must be completed and signed by a licensed physician, physician assistant or nurse practitioner. Part 3 asks for the practitioner's certificate or license number, specialty, mailing address, state where licensed and telephone number, then a full signature and date.

Can I attach a letter to the DS-6?

Yes. After choosing whether the patient should be evaluated by DMV or whether driving privileges should be suspended, Part 2 lets you give further detail in the space provided or in an attached statement on your letterhead.
