Nebraska DMV 06-19: Statement of Physician

Nebraska's Department of Motor Vehicles uses this two-page statement to decide whether a driver or applicant with a neurological, psychiatric, substance use, diabetic or cardiac condition can be licensed. The examining physician completes the relevant condition sections and a required general statement on mental and physical capability, and the report is not valid more than 90 days after the exam.

Who completes it

The applicant signs the authorization at the top before the exam; the examining physician, an M.D. or D.O., completes the condition sections and general statements and signs.

Submitting and payment

How to submit it
The physician or applicant mails, faxes or emails it to the Nebraska DMV, using the details at the top of the form; it is not valid more than 90 days after the examination date.

Good to know

  • The general statements must be completed in full, including whether the patient is mentally and physically capable of driving, with an option for 'only if appropriate tests as determined by the DMV are passed'.
  • Psychological symptoms are each marked 1 (does not impair driving) or 2 (impairs driving), covering anxiety, delusions, hallucinations, memory, judgment, daytime sleepiness and others.
  • Diabetes questions cover oral agents, insulin, hypoglycemic reactions, renal disease and retinopathy, and ask whether a statement on vision should also be required.
  • For commercial drivers only, the form asks whether the condition existed before July 30, 1996.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
