Nebraska DMV 06-18: Statement of Vision

Nebraska's DMV asks for this two-page eye report when a driver fails the DMV vision screening or is under vision review. The optometrist or ophthalmologist records unaided and best-corrected acuity, telescopic and carrier-lens acuity for bioptic users, Goldmann horizontal fields, diplopia and a retest interval, and the statement is not valid more than 90 days after the exam.

Who completes it

The applicant signs the authorization before the exam; an optometrist or ophthalmologist completes the numbered items and signs, answering items 7 and 8 only for commercial drivers.

Submitting and payment

How to submit it
Mail, fax or email it to the Nebraska DMV, or return it to the driver license examiner as the form's title asks; it is not valid more than 90 days after the exam date.

Good to know

  • If the applicant needs new lenses to reach the best-corrected acuity recorded, delay signing until the new lenses are in use.
  • Give cause and prognosis under Additional Comments if best acuity is below 20/40 in either eye or both, or the total horizontal field is under 140 degrees.
  • Fields must be measured with a III4e or V4e Goldmann target or an equivalent, such as the SSA Kinetic V4e isopter test on Humphrey field analyzers.
  • Bioptic users need acuity through both the telescopic lens and the carrier lens, plus the field through the carrier lens.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When should the eye doctor sign Nebraska's Statement of Vision?

After the applicant is wearing any new lenses needed to reach the best-corrected acuity on page 1; the form says to delay signing until then. The statement is not valid more than 90 days after the examination date, so the exam date must be filled in.

What visual field test does Nebraska accept?

The horizontal field must be measured with a III4e or V4e Goldmann test target or an equivalent, such as the SSA Kinetic V4e isopter test on Humphrey field analyzers. Degrees temporal and nasal are recorded for each eye, and cause and prognosis are needed if the total field is under 140 degrees.

What does the Statement of Vision ask commercial drivers?

Two items apply only to commercial motor vehicle operators: whether a vision condition that existed before July 30, 1996 has significantly worsened or another condition developed, and whether the driver can recognize standard red, green and amber traffic signal colors.
