Minnesota D4: Ophthalmologist Examination Report

Minnesota uses this one-page report when a driver with diabetes applies for a waiver from the medical standards for intrastate school bus drivers. The ophthalmologist answers whether diabetes has affected the patient's eye health, starting with whether there is unstable proliferative diabetic retinopathy, and records acuity, fields and stability.

Who completes it

An ophthalmologist completes and signs the whole form, including the patient's name in the letter at the top and the office or clinic details.

Submitting and payment

How to submit it
Return it to Minnesota DPS Driver and Vehicle Services by mail or fax, using the details printed on the form.

Good to know

  • The examination must have taken place within the preceding six months, and the date of examination goes next to the signature.
  • Five questions: unstable proliferative diabetic retinopathy, Snellen distance acuity in each eye, horizontal fields in degrees, whether readings are with or without lenses, and whether acuity is stable.
  • The form is a flat 2010 PDF with no fillable fields; DVS lists it as the Ophthalmologist Examination Report (D4).
  • It is specific to the intrastate school bus waiver for diabetes. A driver who fails DVS vision screening uses the PS30338 Vision Report instead.
