Illinois PTAX-343-A Physician's Statement (HEPD)

Illinois homeowners applying for the Homestead Exemption for Persons with Disabilities must prove disability, and PTAX-343-A is the fallback when they lack a Class 2 disability ID card or an SSA, VA, railroad or civil service benefit letter. A licensed clinician certifies, using Social Security Administration criteria, that the applicant is disabled.

Who completes it

The applicant completes Step 1 with the property owner's name, address, assessment year and PIN. An Illinois-licensed physician, advanced practice nurse, physician assistant or optometrist (visual impairment only) completes Step 2, Parts A and B, and signs.

Submitting and payment

How to submit it
The applicant files PTAX-343-A with Form PTAX-343 at the Chief County Assessment Office before the county's HEPD due date.
Who pays
The form states that the applicant is responsible for any physician's costs.

Good to know

  • Part A asks the disability onset date, whether the patient can do prior work or worked after that date, and whether the disability has lasted or will last 12 months or more.
  • The clinician checks every affected body system from the SSA Listing of Impairments, 1.00 Musculoskeletal through 14.00 Immune, and describes the nature of the disability.
  • Alcoholism or drug abuse is not included in the SSA guidelines as a qualification for disability status.
  • The county records verification of the signer's license with DFPR; the form is marked web only, one copy, revision R-01/21.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Illinois PTAX-343-A?

A licensed physician, advanced practice nurse, physician assistant or optometrist. Optometrists may certify only disabilities related to visual impairment. The signer gives a license number and issuing state and states the information is true based on the Social Security Administration's criteria for disability.

When is a PTAX-343-A needed?

Only when the applicant cannot provide other proof: a Class 2 or 2A Illinois Person with a Disability ID card, an SSA disability benefit letter, a VA non-service-connected pension letter, or a railroad or civil service letter showing total disability.

Who pays for the physician's statement for the Illinois disability exemption?

The applicant. PTAX-343-A states that applicants are responsible for any physician's costs of completing the form. The completed statement is then filed with the PTAX-343 application at the Chief County Assessment Office before the county's deadline.
