Florida DR-416B Optometrist Disability Certification

Florida homestead owners who are totally and permanently disabled by legal blindness can qualify for property tax exemption with certification from a Florida optometrist. On DR-416B the optometrist certifies the person was totally and permanently disabled as of January 1 due to legal blindness, under Rule 12D-16.002 of the Florida Administrative Code.

Who completes it

A Florida optometrist licensed under Chapter 463 completes and signs the whole certificate, including the patient's name and Social Security number, the optometrist's address, Florida Board of Optometry license number and issue date.

Submitting and payment

How to submit it
The optometrist gives the form to the patient, who presents a copy to the county property appraiser on or before March 1 of each year.
Who pays
The Department of Revenue sets no fee and the form does not say who pays the optometrist.

Good to know

  • DR-416B is limited to legal blindness; Florida's DR-416 certification and the DR-501 exemption application are its companion forms.
  • A letter from the U.S. Department of Veterans Affairs can be presented to the property appraiser instead of this form.
  • Disclosure of the Social Security number is mandatory under sections 196.011(1) and 196.101(7), Florida Statutes, to verify identity and exemption information.
  • Section 196.131(2) makes knowingly giving false information to claim homestead exemption a first-degree misdemeanor, a warning addressed to both taxpayer and optometrist.
