BOE-19-DC Certificate of Disability (CA Prop 19)

California homeowners who are severely and permanently disabled can transfer their property tax base to a replacement primary residence under Proposition 19, and BOE-19-DC supplies the medical certification for that BOE-19-D claim. Part I asks the physician to describe the disability and the disability-related requirements of the replacement home.

Who completes it

A licensed physician or surgeon of appropriate specialty completes Part I: patient's name, date of disability, description, why the disability requires the move, the replacement home's requirements including location, specialty, signature and daytime phone. The claimant, spouse or guardian completes Part II.

Submitting and payment

How to submit it
The claimant files the certificate with the BOE-19-D claim at the county assessor; the BOE site posts only a watermarked sample, so the usable form comes from the assessor.
Who pays
The Board of Equalization sets no fee and the form does not say who pays the physician.

Good to know

  • The physician certifies the patient meets the Revenue and Taxation Code section 74.3 definition of a severely and permanently disabled person.
  • In Part II the claimant certifies under penalty of perjury that the move either satisfies the Part I requirements or alleviates financial burdens caused by the disability.
  • The completed certificate is marked as not subject to public inspection, which protects the medical details the physician provides.
  • The current revision is REV. 03 (05-25); the older BOE-62-A serves section 69.5 transfers under the earlier rules.

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Where do I get a usable BOE-19-DC form?

From the county assessor. The Board of Equalization website shows a watermarked sample marked contact assessor for actual form, so the physician should complete the copy the claimant obtains from the assessor's office rather than the online sample.

Who can sign the California Certificate of Disability BOE-19-DC?

A licensed physician or surgeon of appropriate specialty. The physician states their specialty, describes the patient's disability and its effect on housing needs, and certifies that in their medical opinion the patient qualifies as a disabled person under Revenue and Taxation Code section 74.3.

What does the physician need to describe on BOE-19-DC?

Two things beyond the disability itself: the specific reasons the disability necessitates a move to the replacement primary residence, and the disability-related requirements, including any locational requirements, of that residence. The claimant then explains how the new home meets them.
