Arkansas AR1000-DD Developmental Disability Certificate

Arkansas taxpayers claiming the individual income tax credit for a dependent with a developmental disability attach AR1000-DD, formerly AR1000RC5, to their return. A physician, psychologist or psychological examiner certifies a diagnosis of autism, cerebral palsy, Down syndrome, epilepsy or intellectual disability that originated before age 22.

Who completes it

The taxpayer enters names and Social Security numbers, certifies the dependent is not claimed by another taxpayer and authorizes the Department to contact the certifier. A licensed physician, psychologist or psychological examiner checks the diagnosis, answers the onset and impairment questions and signs with name and NPI.

Submitting and payment

How to submit it
The certificate is attached to the taxpayer's Arkansas individual income tax return.
Who pays
The Department of Finance and Administration sets no fee and the form does not say who pays the certifier.

  • Physician signature and address stamps are acceptable on the doctor or examiner's certification section of the form.
  • The certifier gives the initial diagnosis date and confirms the condition began before age 22 and substantially impairs functioning without support services such as therapy or job training.
  • By signing, the taxpayer authorizes DFA to request records from the certifier to verify the diagnosis and other information on the form.
