Alabama PT-PA-1 Physician's Affidavit of Disability

Alabama homeowners claiming the H-3 total homestead exemption from state and county property tax because of disability need a treating physician's sworn statement. On PT-PA-1 the physician confirms active treatment of the disability, gives an opinion that the person is permanently and totally disabled, and states the date the disability began.

Who completes it

An Alabama-licensed physician actively providing treatment directly related to the disability completes and signs the affidavit, including license number with issue and expiration dates. The top section identifies the person examined by name and address.

Submitting and payment

How to submit it
The physician gives the signed affidavit to the homeowner, who files it with the homestead exemption at the county revenue office.
Who pays
The Department of Revenue sets no fee for the affidavit and it does not say who pays the physician.

Good to know

  • The physician certifies having personally examined the person's physical condition and determined them to be permanently and totally disabled.
  • The affidavit quotes Code of Alabama 40-9-21.2: false information to claim or help claim a homestead exemption can trigger twice the tax due, retroactive up to 10 years, plus interest.
  • Two yes or no questions open the form: whether the physician is actively treating the disability and whether, in their opinion, the person is permanently and totally disabled.
  • The license block asks for a current Alabama medical license number with its issue and expiration dates, plus business address and phone.
