HCAD GTA:IAD:013: Physician's Disability Statement

Harris Central Appraisal District uses this statement to document disability for the Texas disability residence homestead exemption when the homeowner does not receive Social Security disability. The physician confirms the owner cannot engage in substantial gainful activity for at least 12 months, or is blind and over 55, and gives the onset date. The physician mails it directly to the HCAD Exemption Center.

Who completes it

The property owner completes Part A: name, property address or legal description and the years the form covers. The physician completes Part B: length of treatment, last exam, when the applicant last worked and may return, the condition in plain language, the disability date, license number and signature.

Submitting and payment

How to submit it
The physician must mail the completed form to the HCAD Exemption Center; HCAD will not accept it if the owner simply attaches it to the homestead application.
Who pays
The form lists no fee or billing code for the physician's statement.

Good to know

  • Owners receiving federal Social Security disability benefits qualify automatically and usually send their SSA determination letter instead; benefits from other disability programs do not automatically qualify.
  • The owner still files the Application for Residential Homestead Exemption and marks the disability box; inadequate documentation is the most common reason HCAD denies this exemption.
  • HCAD usually acts within four to six weeks of receiving the application, and an owner who is denied can protest the decision.
  • This is a Harris County form. Texas has no statewide physician form, and other appraisal districts publish their own versions.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can the homeowner submit the HCAD physician's statement?

No. The instructions say the physician must mail the completed form to the Harris Central Appraisal District Exemption Center. A statement simply attached to the homeowner's homestead application will not be accepted. The owner completes Part A first and then gives the form to the physician.

What definition of disability does HCAD use?

The Social Security test: a medically determinable physical or mental impairment that prevents substantial gainful activity and is expected to last at least 12 continuous months or result in death. A person over 55 who, because of blindness, cannot do the gainful work they previously did with some regularity also qualifies.

What must the physician answer in Part B?

How long they have treated the disabling condition, the last exam date, when the applicant last worked and is expected to return to work, and a plain-language description of the condition. The physician then states the date the owner became 100% disabled and unable to be gainfully employed, and signs with a license number.
