WeGo Access Application: Professional Certification

WeGo Public Transit in Nashville requires certification before anyone can ride WeGo Access, its ADA paratransit service. Section 2 of the application asks a physician or other licensed professional to certify a severe disability, give the diagnosis, onset and prognosis, and rate the patient's ability to perform ten travel tasks. The applicant brings the completed application to an in-person interview and functional assessment.

Who completes it

The applicant, or anyone helping them, completes Section 1 and signs a release naming the professional. A licensed professional completes the Certification of Disability: for example a physician, physiatrist, ophthalmologist, psychologist, registered nurse, physical or occupational therapist, audiologist, clinical social worker or mobility specialist.

Submitting and payment

How to submit it
WeGo does not take the application by mail or fax: the applicant schedules an in-person interview and functional assessment and brings the completed application and their mobility aids.

Good to know

  • The certification asks for DSM-IV codes for cognitive or psychiatric disabilities, whether a seizure disorder or psychiatric condition is controlled by medication, and a visual acuity statement for visual disabilities.
  • Task ratings include traveling a quarter mile to a stop, waiting ten minutes without a seat, climbing three 10-inch steps, crossing streets safely and traveling in heat, cold, ice or snow.
  • WeGo decides within 21 days of a complete application, interview and assessment: fully eligible, conditionally eligible or not eligible, with an appeal open to those found ineligible.
  • The application must be completed in English, typed or clearly printed, although WeGo offers it in large print, Braille and audio formats.

Frequently asked questions

Who can sign the WeGo Access Certification of Disability?

A licensed professional who can verify the disability. The form's examples are clinical social workers, independent living specialists, occupational and physical therapists, physiatrists, rehabilitation specialists, audiologists, ophthalmologists, physicians, psychologists, registered nurses and mobility specialists, and it notes the list is not complete. The professional signs with profession and license number.

Does a professional's certification guarantee WeGo Access eligibility?

No. The application states that verification from a licensed health care professional does not automatically qualify the applicant. Under the ADA, disability alone is not enough: WeGo bases the decision on whether the person can use regular buses, using the interview, the functional assessment and the professional's ratings.

What happens after the WeGo Access application is complete?

The applicant schedules an in-person interview and functional assessment, which takes about 45 minutes to an hour and may include going outdoors. WeGo provides transportation to the interview at no cost if needed. A decision follows within 21 days of the complete application, interview and assessment.
