Vermont VL-038: Temporary Disabled Parking Placard

Vermont DMV uses VL-038 to issue a temporary disabled parking placard to someone with a short-term ambulatory handicap. The one-page form has an applicant section and a clinician section certifying that the person is temporarily disabled and recommending when the placard should expire. The maximum is six months, and the placard cannot be renewed.

Who completes it

The applicant completes the top section, including Social Security number, Vermont license number and physical description, and signs under 23 VSA § 304(f). A licensed physician, certified physician assistant or licensed advanced practice registered nurse completes the bottom section with license number, signature and date.

Submitting and payment

How to submit it
The applicant submits the completed form to the Vermont DMV.

Good to know

  • The clinician recommends an expiration month and year; the form caps validity at six months and states the temporary placard may not be renewed.
  • Permanent placards use a different application, VD-120, and Vermont DMV asks a physician to complete VS-113, the Medical Evaluation Report, for permanent placards or disabled plates.
  • The applicant affirms under penalty of perjury and acknowledges the limits on use of the placard by anyone other than the disabled person.
  • The current version is VL-038 dated 03/2019; the DMV forms index lists it as Temporary Disabled Parking.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign Vermont's temporary disabled parking form?

A licensed physician, a certified physician assistant or a licensed advanced practice registered nurse. The clinician checks that the person is temporarily disabled with an ambulatory handicap, recommends the month and year the placard should expire, and prints their name and license number before signing and dating.

What if the patient needs a placard for longer than six months?

VL-038 states that the temporary placard is valid for six months at most and may not be renewed. For a long-term condition, Vermont's permanent placard application is VD-120, which the DMV pairs with a physician's VS-113 Medical Evaluation Report.
