Transit Plus Medical Verification Form (Milwaukee)

Milwaukee County's Transit Plus, the MCTS ADA paratransit service, uses a two-step application. Step 2 is the Medical Verification Form, which only a licensed healthcare professional may complete, supporting the applicant's Step 1 application. Once both steps are complete, the Transit Plus office contacts the applicant to schedule an in-person functional assessment.

Who completes it

The applicant or a designee completes Step 1, the Application for Transit Plus Services, then contacts their healthcare provider. A licensed healthcare professional completes the Step 2 Medical Verification Form.

Submitting and payment

How to submit it
The professional completes Step 2 through MCTS's online form link or on a printable PDF; paper forms are returned in person or by mail to the Transit Plus Office in Milwaukee.

Good to know

  • Incomplete applications delay the eligibility decision, and the applicant or designee is responsible for making sure every required part is filled in.
  • Step 1 can be completed online or on a printable application available in English and Spanish.
  • Applicants who may need personal care, toileting help or behavioral supervision must bring a caregiver to the in-person assessment; the office can arrange transportation to it.
  • Transit Plus clients also qualify for MCTS's Reduced Fare ID by showing their Transit Plus ID, without the TP51 provider section.

Who can complete the Transit Plus Medical Verification Form?

MCTS states that only licensed healthcare professionals are qualified to complete the Medical Verification Form. The applicant or a designee completes the separate Step 1 application, then asks their provider to complete Step 2 so the application can be reviewed and the functional assessment scheduled.

How does a provider submit the Transit Plus medical verification?

MCTS offers the Medical Verification Form as an online form the professional fills in directly, and as a printable PDF. Paper versions go back to the Transit Plus Office in person or by mail. Either way, the application is not reviewed until both steps are complete.
