Wisconsin MV2548: Permanent Disabled Parking Permit

Wisconsin's MV2548 is the application for a permanent Disabled Parking Identification (DIS ID) permit under s. 343 Wis. Stats. A health care specialist meets it in the Eligibility section, certifying that the applicant has a permanent qualifying disability. The permit hangs from the rearview mirror when parked and can be used in any vehicle carrying the holder.

Who completes it

The person with the disability, or someone signing on their behalf, completes the Applicant section. A health care specialist licensed in any state completes the Eligibility section: physician, podiatrist, advanced practice nurse, chiropractor, public health nurse, PA, physical therapist, or a Christian Science practitioner residing in Wisconsin.

Submitting and payment

How to submit it
The applicant mails the form to WisDOT's Special Plates Unit or takes it to a DMV service center that offers DIS ID service; specialists can also certify online through WisDOT's disabled parking site.
Who pays
WisDOT charges no fee to issue, renew or replace a permanent DIS ID permit, although DMV service centers add a counter service fee.

Good to know

  • The specialist enters the date of visit, and the certification is not valid more than 12 months after that date.
  • Permanent DIS ID holders must be recertified every four years to renew; temporary disabilities use form MV2933 instead.
  • A registered nurse needs additional credentials to certify eligibility, and the statement is not to be considered a claim for VA benefits.
  • Qualifying criteria include inability to walk 200 feet without resting, needing an assistive device, portable oxygen, AHA Class III or IV cardiac limits, and severe arthritic, neurological or orthopedic limits.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Who can sign Wisconsin's permanent disabled parking permit form?

A health care specialist licensed to practice in any state: physician, podiatrist, advanced practice nurse, chiropractor, public health nurse, licensed or certified physician assistant, or physical therapist, or a Christian Science practitioner residing in Wisconsin. Registered nurses need additional credentials. The signer certifies a permanent qualifying disability.

How often must a Wisconsin permanent DIS ID be recertified?

Every four years. The form states that individuals with a permanent disability must be recertified every four years to renew the permit, so patients return to a health care specialist for a new Eligibility section at that point, dated within 12 months of the visit.

Is MV2548 available in Spanish?

Yes. WisDOT publishes MV2548S, the Spanish version (Permiso de Estacionamiento para Personas con Discapacidad Permanente). Health care specialists can also skip the paper Eligibility section and certify the applicant's qualifying disability online through WisDOT's disabled parking site.
