Wisconsin MV2162: Disabled Parking License Plates

Wisconsin's MV2162 is the application for disabled parking license plates under s. 341.14(1a) Wis. Stats. A health care specialist meets it when the health care certification section needs confirmation of a qualifying permanent disability, based on an exam in the last 12 months. The applicant can also request one Disabled Parking Identification (DIS ID) permit on the same form.

Who completes it

The applicant completes the applicant, vehicle and plate sections. Health care specialists licensed in any state complete the certification: physician, podiatrist, advanced practice nurse, chiropractor, public health nurse, PA, physical therapist, or a Christian Science practitioner residing in Wisconsin.

Submitting and payment

How to submit it
The applicant mails the application with the required fees to WisDOT's Special Plates Unit; specialists can also certify eligibility online through WisDOT's disabled parking site for faster processing.
Who pays
The applicant pays WisDOT's plate issuance fee and any registration fees due, plus an extra fee for personalized plates.

Good to know

  • Health care certification is not required if the applicant already holds a valid permanent (blue) DIS ID permit; anyone holding two permits returns one.
  • Persons with a permanent disability must be recertified every four years to keep their disabled parking license plates.
  • Qualifying people include the vehicle owner or lessee, someone regularly transported by them, an employee given an employer's vehicle, or a trust beneficiary.
  • If a condition may prevent safe control of a vehicle, the form points clinicians to WisDOT's medical reporting process for health professionals.

Frequently asked questions

Who can certify Wisconsin disabled parking plates?

A health care specialist licensed to practice in any state: physician, podiatrist, advanced practice nurse, chiropractor, public health nurse, licensed or certified physician assistant, or physical therapist, or a Christian Science practitioner residing in Wisconsin. A registered nurse needs additional credentials, and the certification must rest on an exam within 12 months.

Can the health care certification for MV2162 be done online?

Yes. The form tells health care specialists they can certify eligibility online through WisDOT's disabled parking site for faster processing, as an alternative to signing the paper form. The applicant still completes and mails the applicant and vehicle sections with the fees.

Is MV2162 available in Spanish?

Yes. WisDOT publishes a Spanish version, MV2162S (Placas de Estacionamiento para Personas con Discapacidad Información y Solicitud), alongside the English form. The health care certification section asks for the same information in both versions.
