MCTS TP51: Milwaukee Reduced Fare ID Application

Milwaukee County Transit System uses form TP51 to issue Reduced Fare ID cards based on age or disability. When the applicant qualifies through a physical or mental impairment under the FTA definition, the treating physician or a licensed health care provider with a Wisconsin medical license completes the back page, checking the qualifying criteria and confirming the patient is under their care.

Who completes it

The applicant completes the front, picks the eligibility basis and signs a release of disability-related medical information. A treating physician or licensed health care provider holding a Wisconsin medical license completes the back, including a required state license number.

Submitting and payment

How to submit it
The applicant returns the completed form to MCTS and shows photo ID to receive the Reduced Fare ID.

  • Criteria groups: non-ambulatory, semi-ambulatory (arthritis, limb loss, stroke, cardiopulmonary, dialysis), visual, hearing, cognitive, neurological and other disabilities, with a diagnosis required for some.
  • Pregnancy, obesity, drug or alcohol addiction and controlled epilepsy do not qualify; for epilepsy, a person seizure-free for six months does not qualify.
  • The provider marks the impairment permanent or gives its duration, or checks a box stating it does not meet any listed limitation.
  • Medicare beneficiaries, eligible Transit Plus clients, and riders aged 6 to 11 or 65 and older qualify with proof instead of a provider certification.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
