Kansas TR-159: Certification of Disability for Parking

Kansas residents use TR-159 to apply for a disabled identification placard, disabled license plate or wheelchair emblem decal. The Healing Arts Licensed Professional's Statement certifies that the person meets at least one condition in K.S.A. 8-1,124. A clinician meets it when a patient needs accessible parking, and the county treasurer's motor vehicle office issues the placard, plate or decal.

Who completes it

The disabled individual (or a business or agency representative) completes the top section and signs. An MD, DO, chiropractor, podiatrist, optometrist, physician assistant, ARNP, Christian Science practitioner or physical therapist completes and signs the professional's statement.

Submitting and payment

How to submit it
The applicant takes the completed form to the local county treasurer's motor vehicle office, where all Kansas disabled placard, plate, decal and ID card applications are made.
Who pays
Placards carry no charge and plates a small reflective plate fee paid by the applicant; the form does not address payment for the professional's statement.

Good to know

  • Kansas uses a 100-foot threshold: the patient cannot walk 100 feet without stopping to rest, or is severely limited in walking 100 feet due to arthritic, neurological or orthopedic conditions.
  • The professional must check permanent or temporary; temporary certifications need from and to dates, and six months is the maximum for a temporary placard.
  • Only applicants certified as permanently disabled may get a disabled license plate. Lost or stolen placard replacements need no new professional's statement.
  • Rubber-stamped or initialed signatures are not accepted. TR-159a covers self-certification of continued eligibility, and TR-264 is a separate cognitive disorder ID placard that is not for parking.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign Kansas form TR-159?

Only MDs, DOs, chiropractors, podiatrists, optometrists, licensed physician assistants, ARNPs registered under K.S.A. 65-1131, Christian Science practitioners listed in The Christian Science Journal, and physical therapists registered under K.S.A. 65-2901. The form states that an RN or LPN is not authorized to certify or sign it.

Can an out-of-state doctor sign the Kansas TR-159?

Yes. The form states that it may be signed by a healing arts professional licensed in any state. The applicant must still be a Kansas resident and apply through their local county treasurer's motor vehicle office.

Six months is the maximum duration for a temporary placard. The professional enters from and to dates on TR-159, and a temporarily disabled person may be issued one or two temporary placards. A longer need requires a new certification.
