MBTA The RIDE: Health Care Provider Information

The RIDE is the MBTA's ADA paratransit service in the Boston area. Eligibility is decided mainly through an application and an eligibility appointment at the MBTA Mobility Center, so there is no standard provider form. A clinician meets this step when the Mobility Center asks for additional information from the applicant's health care provider, and the application is not complete until that information arrives.

Who completes it

The applicant applies online or on a paper application mailed on request, then attends the eligibility appointment. The applicant's health care provider answers the Mobility Center's specific request, if one is made.

Submitting and payment

How to submit it
There is no download: the provider responds to the MBTA Mobility Center's request for that patient as directed in the request. The Mobility Center decides within 21 days after the application is complete.
Who pays
The MBTA does not address payment for provider information requests.

Good to know

  • The eligibility appointment is usually in person at the Mobility Center in Boston, and some applicants have a physical assessment led by an occupational or physical therapist.
  • The request for provider information can come after the appointment. Because the clock starts when the application is complete, a quick reply shortens the patient's wait.
  • Decisions are unconditional (any trip), conditional (some trips under stated conditions) or ineligible, and applicants can appeal using the instructions in the decision notice.
  • A licensed health care provider can also request 30 days of RIDE service starting immediately after a medical emergency.

Get the note ready with Vero

There is no form to fill: the program asks for a letter or report with the content it sets. Vero Scribe writes the visit note it draws on, so the details are ready when you write it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there an MBTA RIDE form for doctors to fill out?

The MBTA does not publish a standalone provider form for The RIDE. Applicants complete the application and an eligibility appointment, and the Mobility Center may then request additional information from the applicant's health care provider for that specific case.

How long does RIDE eligibility last?

For most customers, RIDE eligibility lasts three years. Riders periodically confirm their ADA status and receive a reminder before eligibility expires. Approved riders can also request proof of MBTA ADA paratransit eligibility to use paratransit services outside the Boston area.

Can a provider get a patient on The RIDE right after an emergency?

Yes. The MBTA lets a licensed health care provider request 30 days of RIDE service that starts immediately after a medical emergency. This bridges the gap while the patient goes through the regular application, appointment and decision process.
