Massachusetts MAB100: Disabled Parking Placard or Plate

Massachusetts residents apply on MAB100 for a temporary or permanent disability placard, a disability plate, a Disabled Veteran plate or a hospice care placard. The healthcare provider writes a clinical diagnosis, checks the qualifying criteria, such as being unable to walk 200 feet without stopping to rest, and answers whether the patient is medically qualified to drive. RMV Medical Affairs returns incomplete applications unprocessed.

Who completes it

The applicant completes Sections A to C. The healthcare provider completes Section D (driving fitness, diagnosis, duration and criteria) and Section E (NPI, Board of Registration in Medicine number and signature); MDs, osteopaths, PAs, registered nurses, chiropractors, podiatrists and optometrists (legal blindness only) may sign.

Submitting and payment

How to submit it
The applicant mails the signed original to RMV Medical Affairs within 30 days of the provider's certification; faxes are not accepted and service centers do not process it. The RMV asks applicants to allow at least 30 business days.
Who pays
Placards are free and plates carry registration fees paid by the applicant; the form does not address payment for the provider's certification.

Good to know

  • Symptoms such as pain, or ICD codes alone, are not a clinical diagnosis and lead to denial; write out the condition causing the mobility restriction.
  • Chronic lung disease qualifies at an FEV1 under 1 liter with results attached. Asthma alone does not, and oxygen saturation of 88% or less with minimal exertion means automatic loss of license.
  • Legal blindness and AHA functional Class IV heart disease also mean automatic loss of license; a Certificate of Blindness may substitute for the provider's certification.
  • Disabled Veteran plates need a VA breakdown letter listing each service-connected condition with its percentage, totaling at least 60% from conditions that affect ambulation.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Why do Massachusetts disability placard applications get rejected?

Medical Affairs denies applications that give symptoms such as pain, or only ICD codes, instead of a clinical diagnosis. Asthma alone does not qualify, and lung disease needs pulmonary function results. Missing fields, including the provider's NPI, registration number or the driving question, lead to delays or the application being returned.

Does the provider have to answer the driving question if the patient does not drive?

Yes. The form says the physician must complete the medical qualification question regardless of the patient's license status or age. The options are that the condition will not impair safe driving, that the patient is not medically qualified to drive, or that a competency road test is needed.

How long does a Massachusetts disability placard last?

The provider chooses temporary, with an estimated number of months, or permanent. A permanent placard is mailed again automatically when it expires, while a temporary placard that is expiring or needs recertification requires a new MAB100. A person may hold only one valid placard at a time.
