Kentucky TC 96-347: Disabled Plate or Parking Placard

Kentucky residents use TC 96-347 to apply for a disabled parking placard or a disabled parking registration plate. The applicant completes Section 1 and either Section 2 (placard) or Section 3 (plate), never both. A clinician meets the form when a patient needs certification that a severe impairment limits mobility, and the patient presents it to the county clerk.

Who completes it

The applicant completes Section 1, with their signature attested by a notary or attesting official. A county clerk, licensed physician, physician assistant, physical therapist, occupational therapist, chiropractor or APRN signs the Section 2 or Section 3 certification.

Submitting and payment

How to submit it
The applicant presents the completed form to their county clerk. The certifier's signature date is valid for 60 days, so the patient should file soon after the visit.
Who pays
The form does not list a fee or address payment for the certification.

Good to know

  • Section 2 offers a permanent blue placard valid six years (KRS 189.456) or a temporary red placard valid three months (KRS 189.458).
  • The placard certification covers severe visual, audio or physical impairment, including partial paralysis, lower limb amputation, chronic heart condition, emphysema, arthritis, rheumatism or another condition limiting mobility.
  • Section 3, for a plate, certifies the applicant is severely limited in their ability to walk as defined in KRS 186.042(1)(a) through (f).
  • The current version is Rev. 09/2026. A replacement placard section lets the applicant swear the original was lost, stolen or destroyed; disabled veterans use TC 96-217 for plates.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
