Miami-Dade STS Paratransit Application, Part III

Special Transportation Service (STS) is Miami-Dade County's ADA paratransit service for people who cannot use Metrobus, Metrorail or Metromover on their own. A physician meets Part III when a patient applies for STS eligibility. The county uses the physician's answers and attached medical records to decide when the applicant can ride fixed routes and when shared-ride STS is needed.

Who completes it

The applicant, or someone assisting, completes Part I (applicant section) and Part II (release of information). A Florida-licensed physician completes and signs Part III, Medical Verification, with a Florida license number.

Submitting and payment

How to submit it
The applicant sends the complete application with medical documentation to the STS certification office by email, fax or U.S. mail. The STS Certification Unit mails a determination within 21 days.
Who pays
The application does not address payment for completing the medical verification.

Good to know

  • Part III asks the physician to attach objective findings, such as EEG or neuropsychological evaluation with FSIQ, Snellen or visual field reports, CD4 counts, imaging results or FVC/FEV1.
  • Missing documentation delays the decision and prompts the county to contact the physician's office. Blank sections or a missing signature make the application unacceptable.
  • The physician rates whether the applicant can walk to and wait for the bus, board with a ramp, see signs, understand fares and transfer between routes or to Metrorail.
  • Part III also records chemotherapy, radiation or dialysis schedules, the mobility aid used, ambulatory versus wheelchair-van needs, and whether a personal care attendant should ride along.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete the medical verification on the STS application?

Part III must be completed and signed by a Florida-licensed physician, who prints their name and Florida license number and checks a box confirming that objective medical documentation is attached. The form does not list other professions as certifiers for the STS medical section.

What records should be attached to the Miami-Dade STS application?

The county asks for objective tests that substantiate the disability, for example EEG or neuropsychological testing, visual acuity and field charts, CD4 counts, X-ray, MRI or CT findings, and pulmonary function results. Evaluations, notes and reports that explain limits on independent transit use are also accepted.

Is the STS application available in Spanish or Creole?

The application is published in English, with a line at the top directing Spanish and Haitian Creole speakers to the county's paratransit phone line for help. Copies in accessible formats are available on request from the Paratransit Customer Service Office.
