HART Discount Permit Application (Disabled), Tampa

Hillsborough Area Regional Transit (HART) offers a discounted fare on its local bus routes to riders who meet the U.S. Department of Transportation definition of disability. A clinician meets this two-page application when a patient asks for certification of a physical or mental disability. HART verifies the answers before issuing the discount permit.

Who completes it

The applicant completes Section 1 (contact details, rehabilitation or treatment status, wheelchair lift use, temporary or permanent disability). A State of Florida certified physician or medical professional completes all of Section 2 and gives a license number.

Submitting and payment

How to submit it
The applicant brings the completed original to a HART Customer Service outlet (Marion Transit Center or University Area Transit Center) with a valid photo ID. HART does not accept copies, mailed or faxed applications.
Who pays
The applicant pays HART a small permit fee, also charged for replacements; the form does not address payment for the provider's certification.

Good to know

  • Section 2 asks whether the provider personally examined the client, and if not, how the disability was verified, so case managers and rehabilitation programs can also certify.
  • No application is needed for riders holding a red, white and blue Medicare card, a Florida Division of Blind Services card, or a VA disabled ID marked 'Service Connected'.
  • Recertification is required every five years for permanent disabilities and sooner for temporary ones, so the provider's permanent or temporary answer sets the permit's life.
  • HART's instruction sheet reprints the federal definition of disability (49 CFR 37.3), covering physical and mental impairments that substantially limit one or more major life activities.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the HART discount permit application?

Section 2 must be completed by a State of Florida certified physician or medical professional, who prints their name, office or agency and license number. The section must be filled out completely, including the signature, or HART will not issue the permit. Out-of-state licenses are not provided for on the form.

Can the HART discount permit application be faxed or mailed?

No. HART accepts only the original application, presented in person at a HART Customer Service outlet with photo ID. The provider should return the signed original to the patient rather than faxing it to HART, since faxed and photocopied forms are refused.

How long does a HART disabled discount permit last?

HART requires recertification every five years for permanent disabilities. Temporary conditions are recertified sooner, based on the length the provider enters on the form. A lost permit can be replaced for a fee, but a new certification is needed when the permit period ends.
