Indiana State Form 42070: Disability Placard or Plate

Indiana residents use State Form 42070 to apply for a disability license plate or a permanent or temporary disability parking placard. A health care provider certifies the disability on the form. Clinicians meet it when a patient's mobility, breathing, heart condition or vision limits walking, and the Bureau of Motor Vehicles issues the plate or placard from the certified application.

Who completes it

The applicant completes the personal and vehicle details and signs. A health care provider completes the certification section, stating whether the disability is permanent or temporary and, for a temporary disability, the expiration date.

Submitting and payment

How to submit it
The applicant submits the form at any BMV branch or by mail to the address printed on the form, following the BMV's Disability License Plate and Placard Application Mail-In Checklist.
Who pays
The BMV page lists no placard fee and points to regular registration fees for plates; the form's instructions do not address payment for the provider's certification.

Good to know

  • The BMV states that prior versions of State Form 42070 will not be accepted, so download the current form rather than reusing an old office copy.
  • Qualifying conditions include needing a wheelchair, walker, braces or crutches, loss of use of one or both legs, serious lung, heart, arthritic, orthopedic or neurological limits, blindness, and specific diagnosed autism levels.
  • A temporary placard expires on the date the provider lists or one year after issuance, whichever comes first.
  • Disability license plates require a permanent condition. A permanent placard holder can also qualify, as can a vehicle regularly used to transport a qualifying person.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Does an Indiana permanent disability placard expire?

No. A permanent placard does not expire unless a health care provider certifies that the disability is no longer considered permanent. To end it, the provider sends a letter to the BMV Registrations Department saying so. Temporary placards, by contrast, expire on the provider's date or after one year.

Can autism qualify for an Indiana disability placard?

The BMV lists specific diagnosed levels of autism spectrum disorder among the conditions that can qualify for a disability placard, alongside mobility, heart, lung and vision impairments. The health care provider certifies the qualifying condition on State Form 42070.

Where does a patient file Indiana State Form 42070?

At any BMV branch, or by mail using the address on the form. The BMV publishes a mail-in checklist with step-by-step instructions. Plates are renewed through the usual registration channels, including myBMV, kiosks, phone, mail or a branch.
