Nevada SP-27: Disabled Persons Plates and Placards

Nevada residents use SP-27 to apply for disabled persons license plates, placards, or motorcycle and moped stickers. On page 2 a licensed physician, physician assistant, APRN, occupational therapist or physical therapist certifies the qualifying condition and whether it is temporary, moderate or permanent. If that portion is incomplete, Nevada DMV cannot process the application.

Who completes it

The applicant completes page 1 and signs under penalty of perjury. A licensed physician, PA, APRN, OT or PT completes all of page 2, including license number and state.

Submitting and payment

How to submit it
The applicant submits the form at a Nevada DMV office or by mail; first-time applications for disabled plates, motorcycle plates or moped plates must be made in person.
Who pays
Nevada charges no fee for placards, and plate applicants pay registration costs; the form does not address payment for the medical certification.

Good to know

  • Temporary disability means six months or less, and moderate means reversible but lasting more than six months. Both need beginning and ending dates; moderate tops out at two years.
  • Permanent disability certifications are valid indefinitely. Plates require a permanent disability, while motorcycle and moped stickers accept permanent or moderate.
  • Erasures or whiteout void the form, so corrections mean starting over on a fresh copy.
  • Qualifying conditions include walking under 200 feet without rest, assistive device use, AHA Class III/IV cardiac disease, lung function thresholds, severe walking limits, visual disability and portable oxygen.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a physical therapist sign Nevada form SP-27?

Yes. Nevada lets a licensed physician, physician's assistant, advanced practice registered nurse, occupational therapist or physical therapist complete page 2. The certifier checks their provider type, prints their name, license number and state, and signs.

How long is a Nevada disabled placard valid?

It depends on the certification: a temporary disability lasts up to six months and a moderate disability up to two years, each with dates the provider enters. A permanent disability certification is valid indefinitely.

How many Nevada disabled placards can a person get?

Applicants may choose two placards, or license plates plus one placard. Plates are limited to permanent disabilities and require the applicant's name on the vehicle registration and current Nevada insurance.
