SCDMV RG-007A: Disability Placard and Plate Application

South Carolina DMV's RG-007A covers disability placards, disability license plates and the autistic or neurodivergent plate. A clinician meets it when Section C, the Physician's Statement, needs the qualifying conditions checked and the disability marked permanent (placard valid four years) or temporary (at least four months, no more than one year). Applicants file it at a branch or by mail.

Who completes it

The person with the disability, or a parent or legal guardian, completes Sections A and B, and the vehicle owner completes Section D for plates. A licensed physician, APRN, PA or physical therapist completes Section C; physical therapists may certify temporary disability placards only.

Submitting and payment

How to submit it
The applicant takes the form to any SCDMV branch office or mails it with a check or money order; each person with a disability needs a separate application.
Who pays
The applicant pays SCDMV's placard and plate fees with the application, and mailed applications cannot include cash.

Good to know

  • Qualifying conditions include inability to walk 100 feet nonstop without aggravating a condition, AHA Class III or IV cardiac limits, portable oxygen, blindness, and arthritic, neurological or orthopedic walking limits.
  • A cardiac patient whose status improves, for example after bypass surgery or a transplant, no longer meets the cardiac criterion.
  • The autistic or neurodivergent plate requires Form MV-95 and carries no handicapped parking designation. Disability plates require a permanent disability; military plates also need Form MV-37.
  • Only one placard is issued per applicant, and its registration certificate must stay with the person when the placard is used. Permanent placard holders can renew online.

Frequently asked questions

Can a physical therapist sign South Carolina's RG-007A?

Yes, but only for a temporary disability placard. Section C may be completed by a licensed physician, an advanced practice registered nurse, a physician assistant or a physical therapist, and the form states twice that a physical therapist can only certify a person as temporarily disabled. Plates need a physician, APRN or PA.

How long is a South Carolina disability placard valid?

A placard for a permanent disability is valid for four years. For a temporary disability, the clinician states the length of time in Section C, and the impairment must last at least four months but not more than one year. Permanent placard holders can renew online.

Which APRNs can complete the RG-007A Physician's Statement?

The form defines APRNs as nurse practitioners, certified nurse-midwives, clinical nurse specialists and certified registered nurse anesthetists. Any of them may complete Section C, check the qualifying conditions, mark the disability permanent or temporary, and sign with a professional license number.
