San Diego MTS Reduced Fare Long Form (Disability)

San Diego MTS gives a reduced fare and a photo PRONTO card to riders with a qualifying medical disability. People who already have Medicare, SSI or SSDI, a DMV disabled placard, MTS Access eligibility or a qualifying VA letter use the Short Form; everyone else uses this nine-page Long Form, where a treating clinician certifies one of 15 disability categories in Section 2.

Who completes it

The applicant completes and signs Section 1 first. Only the treating physician or qualified health care professional may complete Section 2: the patient's categories, card duration, medication control, impact on transit use, and an original signature with license number, certifying they are licensed in California.

Submitting and payment

How to submit it
The applicant returns the original application in person or by mail to the MTS SDM Eligibility Office. MTS mails a decision within 15 working days once the information and the clinician's certification are verified.
Who pays
The applicant pays MTS a processing fee when picking up a new or renewal card; no fee is set for the clinician's certification.

Good to know

  • MD and DO physicians may certify all 15 categories; psychiatrists and psychologists 10 to 15; audiologists 9; podiatrists 1, 2, 6 and 7; certified school psychologists 15. PAs and NPs certify where licensed to diagnose.
  • Copies, faxed, stamped or typed Section 2 entries are not accepted, and MTS calls the clinician to verify the certification.
  • Reduced fares are not issued for pregnancy, obesity, or drug or alcohol addiction alone, or for a condition controlled by medication. Epilepsy seizure-free for six months does not qualify.
  • The clinician chooses a card duration of 3 months, 6 months, 1 year or 3 years. The form is Rev. #12 (3/2023), with a Spanish version.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify the San Diego MTS reduced fare Long Form?

Only the clinician currently treating the qualifying disability, licensed in California. Physicians (MD or DO) may certify all categories; other professionals are limited, for example audiologists to hearing and podiatrists to nonambulatory, arthritis, amputation and mobility-aided categories. Physician assistants and nurse practitioners may certify categories they are licensed to diagnose.

When should a rider use the MTS Short Form instead of the Long Form?

If the rider is 65 or older, receives SSI, SSDI or Medicare, has DMV disabled eligibility or MTS Access eligibility, has a VA letter showing at least 50% service-connected disability, or holds another California transit disabled ID. The Short Form needs no clinician; the Long Form is for everyone else.
