SacRT ADA Paratransit Professional Verification Form

Sacramento Regional Transit requires this one-page professional verification with every application for SacRT GO ADA paratransit service. A qualified professional lists each diagnosis that keeps the applicant from independently using a lift-equipped bus or light rail train, and explains how. Without it, the application is returned and processing is delayed.

Who completes it

A professional, never the applicant, completes and signs the whole form: a physician, PA, RN or nurse practitioner, psychiatrist, psychologist, social worker, chiropractor, physical or occupational therapist, certified orientation and mobility specialist, or speech-language pathologist. The applicant completes the separate eligibility application.

Submitting and payment

How to submit it
The applicant uploads the signed form in Step 2 of SacRT's online application as a JPG, PDF or PNG up to 5 MB, or mails it with the paper application.
Who pays
SacRT sets no fee for the professional verification.

Good to know

  • The form expires 90 days from the signature date, and an application that arrives without a completed, signed verification is treated as incomplete and returned.
  • Eligibility rests on functional ability, not difficulty or inconvenience. Distance from transit, ability to drive, language and age are not considered.
  • The professional states whether the condition is temporary and for how many months, and whether the person can self-supervise daily activities.
  • Page 2 is a completed example showing the level of detail SacRT expects for diagnoses and functional explanations. The current version is revised August 2025.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the SacRT paratransit professional verification?

Any one of the listed professionals: physician, physician assistant, registered nurse or nurse practitioner, psychiatrist, psychologist, social worker, chiropractor, physical therapist, occupational therapist, certified orientation and mobility specialist, or speech-language pathologist. They check their title, certify under penalty of perjury and give a license, registration or certification number with state.

How long is the SacRT professional verification valid?

The form states that it expires 90 days from the signature date, so a verification signed more than 90 days before SacRT reviews it is out of date. An application without a completed and signed verification is returned to the applicant, which delays the eligibility decision.
