San Diego MTS Access Part B Provider Certification

San Diego MTS decides eligibility for MTS Access, its door-to-door ADA paratransit service, from the applicant's Part A, an in-person assessment and this five-page Part B. The licensed provider describes how the applicant's conditions affect boarding, navigating and riding the accessible bus and trolley system. MTS stresses that diagnosis, age, distance to a stop or inconvenience are not the basis of eligibility.

Who completes it

A licensed provider completes every page: an MD or DO, RN, psychologist, psychiatrist, ophthalmologist, optometrist (visual disabilities only), physical or occupational therapist, or another licensed provider familiar with the condition. The applicant completes Part A separately.

Submitting and payment

How to submit it
Part B is returned with Part A to the MTS Access Eligibility Office by mail or in person, or completed through the MTS application portal. It must be signed within 90 days of the in-person assessment.
Who pays
MTS sets no fee for Part B.

Good to know

  • Question 1 asks for written diagnoses with ICD-10-CM or DSM codes, and question 4 asks whether the disability is short term (at least 90 days, likely to improve within a year) or long term.
  • Questions 10 to 22 ask yes, no or sometimes about heat, rain, air quality, balance, memory, breathing, endurance, hills, vision, hearing, behavior, safety and attendant needs, with explanations.
  • Every question must be answered and legible; incomplete forms are returned to the applicant. MTS and its eligibility contractor may contact the provider for clarification.
  • Part B is available in English and Spanish, revised 12/01/2025. MTS decides within 21 days of the in-person assessment.
