LA Access Services Paratransit Healthcare Verification

Access Services, the ADA paratransit provider for Los Angeles County, decides eligibility through an in-person Transit Evaluation of the applicant's ability to use accessible buses and trains. If the evaluator needs clarification, Access asks the treating professional named in the application to verify the applicant's condition. There is no standalone form to download: the request is made case by case after the evaluation.

Who completes it

The applicant completes the Access Application, including the Healthcare Professional Contact Information section, and attends the evaluation. The treating licensed professional named there, such as a physician, RN, psychiatrist, psychologist, ophthalmologist, optometrist, PT or OT, responds only if the evaluator requests verification.

Submitting and payment

How to submit it
The evaluator tells the applicant at the evaluation if verification is needed, and the professional returns the requested information to Access Services Eligibility. Access mails its decision within 21 days of the Transit Evaluation.

Good to know

  • Access says eligibility depends on the person's ability to use accessible buses and trains, not solely on the disability, age or medical diagnosis.
  • Applicants are asked to bring documents, reports and a list of medications to the evaluation that explain how the disability prevents them from using buses or trains.
  • If eligibility is not processed within 21 days of the evaluation, Access lets the applicant use the service until the notification arrives.
  • An applicant who disagrees with the decision may appeal in writing within 60 days of the eligibility notice; appeal forms are available in English and Spanish.

There is no form to fill: the program asks for a letter or report with the content it sets. Vero Scribe writes the visit note it draws on, so the details are ready when you write it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there a healthcare professional form for LA Access Services?

No standalone form is published. The Access Application only asks the applicant for their healthcare professional's contact information. If the evaluator needs verification after the in-person Transit Evaluation, Access informs the applicant at that time and contacts the named professional for the specific information needed.

Does a doctor's letter make someone eligible for Access paratransit?

Not by itself. Access Services determines eligibility through an in-person Transit Evaluation of the person's ability to use accessible buses and trains. A treating professional's verification can clarify the condition when the evaluator requests it, but the diagnosis alone does not decide eligibility.
