MARTA Reduced Fare Health Care Provider Certification

MARTA's Reduced Fare Card gives people with a mental or physical disability a lower fare on regular bus and rail service. A clinician meets this three-page form when a patient lacks other proof of disability, such as Medicare or Social Security documentation. The provider certifies one of 12 eligibility categories on page 3, and MARTA's Reduced Fare office verifies it before issuing the card.

Who completes it

The applicant completes page 1, including the Breeze Card affidavit and release. A Georgia-licensed physician, psychiatrist, psychologist, ASHA-certified audiologist, physician assistant, advanced registered nurse practitioner or optometrist completes page 2.

Submitting and payment

How to submit it
The applicant brings the completed form to MARTA's Reduced Fare office in person, within 30 days of the provider's original signature and date. Photocopies are not accepted.
Who pays
MARTA issues the first Reduced Fare Card free; the form does not address payment for the provider's certification.

Good to know

  • The provider enters a category number and type from page 3; categories 5, 6, 10, 11 and 12 also need a specific diagnosis written on the form.
  • Enrollment in a drug or alcohol rehabilitation program does not qualify, ADD and ADHD are excluded, and the applicant's finances play no role in eligibility.
  • For asthma under category 6, the provider states whether the patient has been on systemic medication, or used a fast-acting inhaler three or more times weekly, for the past six months.
  • Seniors 65 and older skip this form and show proof of age. Medicare cards, VA service-connected cards and disabled parking permits are also accepted instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign the MARTA Reduced Fare health care provider form?

Only Georgia-licensed providers on MARTA's list: physicians, psychiatrists, psychologists (Ph.D.), audiologists certified by the American Speech-Language-Hearing Association, physician assistants, advanced registered nurse practitioners and optometrists. MARTA states that signatures from other providers are not acceptable.

How long is a MARTA Reduced Fare certification valid?

The signed form must reach the Reduced Fare office within 30 days of the provider's signature. Once approved, MARTA sets the certification period from the documentation, up to three years, and the rider recertifies in person with current documentation when it ends. Temporary disabilities are certified for a stated number of months.

Does mental illness qualify for a MARTA Reduced Fare Card?

Category 11 covers mental illness that includes a substantial disorder of thought, perception, orientation or memory that impairs judgment and behavior. The provider must write a specific diagnosis for this category. Developmental and learning disabilities fall under category 10, which excludes ADD and ADHD.
