MARTA Mobility Part B: Health Care Professional Cert

MARTA Mobility is the ADA paratransit service for people whose disability prevents them from riding MARTA bus or rail. Part B is the professional certification of the applicant's ability or inability to use fixed-route service. MARTA will not process an application until both Part A and Part B arrive, and an in-person interview and assessment follow.

Who completes it

The applicant completes Part A, and a licensed health care professional completes all three pages of Part B, signing with a Georgia state license number. Eligible professionals range from physicians, RNs, NPs and PAs to social workers, psychologists, therapists, chiropractors and counselors.

Submitting and payment

How to submit it
The professional returns Part B to the client. The applicant submits Parts A and B by fax or mail to MARTA Mobility Service, or uses MARTA's online application.
Who pays
The form does not address payment for completing Part B.

Good to know

  • The cover letter explains MARTA's three ADA eligibility categories. Inconvenience and decreased comfort do not qualify; the condition itself must prevent the trip.
  • Part B asks whether the applicant can walk about 200, 300, 600 and 1,200 feet, climb a 12 to 14 inch step, and grip a handrail without assistance.
  • Cognitive and sensory questions cover giving an address, recognizing landmarks, coping with a route change, following directions and moving through crowded facilities.
  • Special education teachers, orientation and mobility specialists, respiratory therapists, speech pathologists and recreation therapists employed by a medical facility are also on the list of eligible certifiers.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete MARTA Mobility Part B?

Part B lists licensed physicians, registered nurses, nurse practitioners, physician assistants, social workers, psychologists, physical, occupational and respiratory therapists, chiropractors, mental health and vocational rehabilitation counselors, special education teachers, orientation and mobility specialists, speech pathologists and recreation therapists employed by a medical facility. A Georgia license number is required.

Does a temporary condition qualify for MARTA Mobility?

Part B asks whether the disability is permanent, and if not, the date the applicant can resume normal travel. It also asks whether the condition is periodic and what triggers flare-ups, so conditional or temporary eligibility can be considered. MARTA makes the final decision after the in-person assessment.

What happens after Part B is submitted?

Once MARTA receives both Part A and Part B, it contacts the applicant to schedule a required in-person interview and assessment. Approved riders then need a MARTA Mobility photo ID Breeze Card to book and board MARTA Mobility vehicles.
