New Hampshire RDMV 130: Walking Disability Privileges

New Hampshire residents use RDMV 130 to apply for walking disability placards or plates under RSA 261:88. In Section II a physician, podiatrist, APRN or physician assistant certifies that the person is under their care and has a walking disability, marked either temporary, for up to six months, or permanent. The applicant then files it with the DMV in person or by mail.

Who completes it

The applicant completes and signs Section I and chooses placards or plates; a third party signing for them must attach guardianship or power of attorney papers. A licensed physician, podiatrist, APRN or physician assistant completes Section II with an original signature.

Submitting and payment

How to submit it
The applicant, or a friend or relative carrying the signed form and their own ID, brings it to any DMV Registration Office, or mails it to the DMV in Concord. Plate renewals go through the town or city clerk.
Who pays
Placards carry no stated fee and first-time walking disability plates a small fee paid by the applicant; the form does not address payment for the provider's section.

Good to know

  • Qualifying criteria include needing an assistive device or person, FEV1 under 1 liter or PaO2 under 60 mm Hg, portable oxygen, AHA class 3 or 4 cardiac limits, or severely limited walking.
  • For a temporary placard the provider writes the number of months, which cannot exceed six. Permanent placards require periodic renewal and recertification under RSA 261:88.
  • A first permanent placard expires at the holder's next license or ID renewal, so it may run under five years; after that it expires every five years with the license.
  • A person may hold two placards, or one placard plus walking disability plates, but not both. Plates require a permanent disability and a copy of the current registration.

How long is a New Hampshire walking disability placard valid?

Temporary placards last up to six months, as the provider specifies. Permanent placards are valid for up to five years and share the expiration date of the holder's driver license or non-driver ID, so the first one may expire sooner, at the next license renewal.
