Maine PS-18: Application for Disability Plates/Placard

Maine residents apply for a disability placard, disability plates or a permanent re-issue with this one-page BMV form, revised 08-14. The medical provider's statement certifies whether the condition is permanent or temporary, six months at most, and checks one qualifying condition. A temporary placard can also cover recovery from childbirth, including cesarean delivery or the birth of a preterm infant.

Who completes it

The applicant completes the top section and signs the statement of understanding. A physician, physician assistant, nurse practitioner or registered nurse completes and signs the Medical Provider's Statement with a medical license number.

Submitting and payment

How to submit it
The completed form can be processed at any BMV branch office or mailed or faxed to the BMV Disability Clerk in Augusta; plate applicants attach their current registration.

Good to know

  • The provider checks one condition, such as inability to walk 200 feet without resting, FEV1 under 1 liter or arterial oxygen under 60 mm Hg at rest, portable oxygen, or AHA Class III or IV.
  • A cesarean delivery placard is valid for 1 week after BMV receives the application; for a preterm birth the provider sets a period of up to 6 months.
  • Permanent credentials expire with the applicant's driver's license or state ID; renewal needs only the top portion marked Permanent Re-Issue, with no new provider statement.
  • BMV charges no additional fee for disability plates or placards, and vehicles displaying them may park at meters without charge for up to twice the time limit.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Can a registered nurse sign Maine's PS-18 disability placard form?

Yes. The Medical Provider's Statement lists physician, physician assistant, nurse practitioner and registered nurse as providers who can certify the condition. The provider prints a name, signs, dates and gives a medical license number.

How long does a Maine temporary disability placard last?

The provider sets the period, up to a maximum of 6 months. For recovery from childbirth, a cesarean delivery placard is valid for 1 week after BMV receives the application, and a placard for the birth of a preterm infant lasts the time the provider specifies, up to 6 months.

Does a permanent placard holder need a new medical statement to renew?

No. Permanent credentials run until the applicant's driver's license or state ID expires. To continue, the applicant completes only the top portion, marks it Permanent Re-Issue and brings it to a BMV branch or sends it to the main office, without returning to the provider.
