Access Pass Physician Statement of Disability

The America the Beautiful Access Pass is a free lifetime pass to federal recreation sites for U.S. citizens and permanent residents with a permanent disability. One accepted proof is a statement signed by a licensed physician. There is no template: the physician writes a signed statement that the person has a permanent physical, mental or sensory impairment substantially limiting one or more major life activities, and names the impairment.

Who completes it

A licensed physician writes and signs the statement. The patient then shows it at a federal recreation site, uploads it with an online order, or mails a photocopy with NPS Form 10-596.

Submitting and payment

How to submit it
The physician gives the signed statement to the patient. In person, an officer at a federal recreation site checks it and issues the pass; online or mail orders include a copy, never the original.
Who pays
The pass is free in person; online and mail orders carry a processing fee plus shipping paid by the applicant, and no fee is set for the physician's statement.

Good to know

  • The statement must attest to a permanent physical, mental or sensory impairment that substantially limits one or more major life activities, and state the nature of the impairment.
  • Instead of a physician statement, applicants can use VA benefit documents, proof of SSDI or SSI, or a state vocational rehabilitation agency document showing blindness or permanent disability.
  • A state DMV disability placard, plate or hang tag is not accepted as proof of disability for the Access Pass.
  • The pass admits the owner and passengers in one non-commercial vehicle, or the owner plus three adults where per-person fees apply. Photo ID is checked whenever it is used.

Read the official requirements

The issuer sets what the letter or report must contain; check its current rules before you write it.

Get the note ready with Vero

There is no form to fill: the program asks for a letter or report with the content it sets. Vero Scribe writes the visit note it draws on, so the details are ready when you write it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What should a physician letter for the Access Pass say?

It must be signed by a licensed physician and attest that the person has a permanent physical, mental or sensory impairment that substantially limits one or more major life activities, such as walking, seeing, hearing or breathing. It must also state the nature of the impairment. No official template exists.

Do I need the NPS application form if I have a physician statement?

Only for mail orders, which use NPS Form 10-596 with photocopies of the statement and proof of citizenship or residency. At a federal recreation site no application is needed: the officer checks the statement and photo ID and issues the free pass. Online orders upload the documents.
