Montana MV5-VSB: Disability License Plate Application

Montana residents use MV5-VSB to apply for disability license plates, which carry the wheelchair symbol, for vehicles they own. Section 3 is the medical certification that the applicant meets the criteria in MCA 49-4-301 and qualifies for five years of disability plates. The applicant files it with the county treasurer, who issues the plates with registration.

Who completes it

The applicant completes Sections 1 and 2. A licensed physician, physician assistant, chiropractor or advanced practice registered nurse completes and signs Section 3.

Submitting and payment

How to submit it
The applicant submits the signed application to the local county treasurer's office, with applicable registration fees.
Who pays
The applicant pays the vehicle registration fees for the plates; the form does not address payment for the medical certification.

Good to know

  • The certification states that the applicant is qualified for five years for disability vehicle plates, so it suits permanent rather than temporary limitations.
  • The criteria match the placard form: walking under 200 feet without rest, assistive devices, portable oxygen, lung or cardiac thresholds, emissions sensitivity, or a comparable documented condition.
  • An unexpired disability placard can be shown at the county office as proof of eligibility for disability plates.
  • Applicants may get one set of plates for each non-commercial vehicle they own. Permanently registered vehicles must carry documentation of continued eligibility.

A licensed physician, physician assistant, chiropractor or advanced practice registered nurse completes Section 3. The certifier signs, dates, prints their name and provider type and gives a professional license number, address and daytime phone.

What is the difference between Montana MV5-VSB and MV5-DSB?

MV5-VSB is for disability license plates and is filed with the county treasurer along with registration fees. MV5-DSB is for a free disability placard and goes to the Motor Vehicle Division. Both use the same MCA 49-4-301 criteria and the same certifier types.
