Montana MV5-DSB: Disability Placard Application

Montana residents use MV5-DSB to apply for a free disability parking placard from the Motor Vehicle Division. The medical provider certifies in Section 1b that the applicant meets one or more criteria in MCA 49-4-301 and chooses the placard length. Clinicians meet it for patients with lasting or temporary mobility limits, including some conditions expected to improve over up to two years.

Who completes it

The individual completes Sections 1 and 1a, and a licensed physician, physician assistant, chiropractor or advanced practice registered nurse completes and signs Section 1b. Organizations complete only Section 2.

Submitting and payment

How to submit it
The applicant sends the signed application to the Motor Vehicle Division by email or mail, or delivers it in person in Helena.
Who pays
There is no fee for a disability placard; the form does not address payment for the medical certification.
