Delaware MV474 Disability Plate and Placard Application

Delaware's Division of Motor Vehicles uses form MV474 for special license plates and parking placards for people with disabilities: a long-term blue placard, an organizational green placard, a plate, or a 180-day red temporary placard. A physician certifies the qualifying condition on page 2 for every first issuance and for each temporary placard renewal under 21 Del. C. sections 2134 and 2135.

Who completes it

The applicant completes Section A first, then Section B for a plate or Section C for a placard. The physician completes Section E (qualifying criteria), Section F for a temporary placard, and the Section G certification with license number, printed name, office address and phone.

Submitting and payment

How to submit it
The applicant submits the original application to Delaware DMV. Photocopies, faxes, and downloaded or scanned applications are not accepted.
Who pays
Initial applications and renewals have no fee, a replacement placard carries a small charge, and no fee is set for the physician's certification.

Good to know

  • Long-term plates and placards are restricted to permanent disabilities with no prognosis for improvement, meeting one of six walking, lung, oxygen, cardiac or orthopedic criteria.
  • A temporary red placard lasts 180 days, and renewing it requires a new application signed by the physician.
  • A physician's signature is not needed to renew a permanent plate or placard, or for applicants 80 or older; they self-certify in Section D.
  • The long-term blue placard is valid for eight years; plates require the person or a household member to own the vehicle, with a 14,000-pound weight limit.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Does a Delaware disability placard renewal need a physician?

Not for a permanent plate or placard, or if the applicant is 80 or older; the applicant self-certifies in Section D that their condition has not changed. A temporary red placard is different: each renewal needs a new application completed and signed by the physician.

What does the physician certify on Delaware form MV474?

In Section E the physician confirms the applicant meets a qualifying criterion, such as being unable to walk 200 feet without rest, needing a device to walk safely, or AHA Class III or IV heart disease. Section G is the signed certification under 21 Del. C. section 2135 with license number.
