Cincinnati Metro Access Medical Verification Form

Access is the ADA paratransit service of Cincinnati Metro (Southwest Ohio Regional Transit Authority). The Medical Verification Form gives a licensed provider's written diagnosis and answers to functional questions supporting the applicant's ADA Transportation Application. Metro states that disability alone, or distance to a bus stop, does not qualify, so the provider's explanation of functional limits carries the application.

Who completes it

A licensed healthcare provider (MD, NP or PA) completes Sections I to III, giving a license or certification number and signing the certification. The applicant completes the separate ADA Transportation Application.

Submitting and payment

How to submit it
The form goes with the digital ADA Transportation Application at ezconnect.org or the PDF application; Access no longer accepts applications by mail or fax. An in-person functional skills assessment follows.
Who pays
The form does not address payment for the medical verification.

Good to know

  • The diagnosis must be written out; the form says 'Do not use codes'.
  • The explanation of how the disability limits use of Metro must be detailed, or the application will not be considered.
  • Yes/no items cover walking 200 feet, walking more than a city block, being left alone on a vehicle, needing a personal care assistant and any history of getting lost or wandering.
  • If the disability is not permanent, the provider gives an anticipated return date. Metro also publishes a Medical Verification Cover Letter to send with the form.

Frequently asked questions

Who can complete the Cincinnati Metro Access Medical Verification Form?

A licensed healthcare provider. The form asks for the provider's title as MD, NP or PA, a license or certification number and hospital or agency affiliation, and the provider certifies that the diagnosis reflects the applicant's current physical or cognitive condition.

Can the Access paratransit application be mailed?

No. Access no longer accepts applications by mail or fax. Applicants use the digital application at ezconnect.org or the PDF application, then schedule an in-person functional skills assessment and bring valid photo ID. A decision is sent within 21 days of the assessment.

What makes an Access medical verification incomplete?

Using diagnosis codes instead of a written diagnosis, or giving a short answer to the question on how the disability substantially limits major life activities affecting use of Metro. The form warns that this question must be answered in detail or the application will not be considered.
