COTA Mainstream Application Part 2 (Columbus, OH)

Mainstream is COTA's ADA paratransit service in Franklin County, Ohio, for people whose functional limitations prevent use of fixed-route buses. Part 2 of the application is the licensed or certified professional's detailed account of those limitations. The applicant brings both parts to an in-person interview and functional assessment, where COTA Mobility Services decides eligibility.

Who completes it

The applicant completes Part 1. A licensed or certified professional who knows the applicant's functional limitations, and is not an immediate family member, completes and signs every section of Part 2 with full name, title, address and license number.

Submitting and payment

How to submit it
Part 2 is not mailed, emailed or faxed: the applicant calls COTA Mobility Services to schedule the interview and assessment and brings both completed parts, which must be done within 90 days of scheduling.
Who pays
The application does not address payment for completing Part 2.

Good to know

  • COTA rejects a Part 2 filled in by the applicant and only signed by the professional. Unanswered questions, no signature or missing license details also delay processing.
  • The diagnosis must be written out ('Please do not use codes'), with the severe symptoms and specific examples of how they prevent independent bus use.
  • A grid rates walking a quarter mile, finding the right bus, climbing three 10-inch steps, using the lift or ramp and following trip instructions, from little difficulty to impossible.
  • Eligible professionals listed on Part 2 include MDs, RNs, chiropractors, O&M specialists, OT, PT, optometrists, psychologists, speech pathologists, LPCCs and LISWs who specialize in the limitation.
