Virginia DMV MED 10: Disabled Parking Placard or Plates

Virginia DMV's MED 10 is the application for a disabled parking placard or disabled parking license plates. A clinician meets it on page 2, certifying that the patient's ability to walk is limited or impaired, or that walking creates a safety concern, and whether the condition is temporary (up to 12 months) or permanent. Permanent placards last five years and renew automatically.

Who completes it

The applicant, parent or legal guardian completes the front, which must be done before the clinician signs. A licensed physician, PA or nurse practitioner completes the medical certification; chiropractors and podiatrists use a separate, shorter list limited to walking criteria.

Submitting and payment

How to submit it
The applicant submits it at any DMV Customer Service Center or DMV Select office, or by mail to DMV; placards are mailed to the applicant.
Who pays
Placards and placard ID cards carry no DMV fee; disabled license plates require plate fees and a VSA 10 application.

Good to know

  • Physician, PA and NP criteria go beyond walking limits to include autism spectrum or other developmental delay that impairs judgment, Alzheimer's or other dementia, and legal blindness or deafness.
  • Permanent means the condition has reached maximum improvement and is not expected to change with further treatment, as tied to Virginia Code §46.2-1240.
  • For a temporary disability the clinician enters beginning and ending dates no more than 12 months apart.
  • Only permanently disabled persons, or institutions transporting people with disabilities, can get disabled plates. Related forms: MED 11 for organizations and MED 12 for program information.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a chiropractor or podiatrist sign Virginia's MED 10?

Yes, using the separate chiropractor or podiatrist certification. It is limited to walking criteria: inability to walk 200 feet without resting, needing an assistive device or another person, severe arthritic, neurological or orthopedic limits, or another described condition. Lung, cardiac, cognitive and vision or hearing criteria are reserved for physicians, PAs and NPs.

How long is a Virginia disabled parking placard valid?

A temporary placard lasts up to 12 months, between the beginning and ending dates the clinician enters. A permanent placard is valid for five years and renews automatically, with the new placard mailed 60 days before expiration, so the patient does not need a new certification for that renewal.

Does Virginia MED 10 cover dementia or autism?

Yes. The physician, PA and NP section includes a mental or developmental delay that impairs judgment, including autism spectrum disorder, and Alzheimer's disease or another dementia. Virginia's application treats a safety concern while walking, not only a physical walking limit, as a qualifying reason.
