Wyoming FSDI-915: Disabled Parking Placard Application

WYDOT Driver Services uses this application to issue disabled parking identification placards to an eligible person under W.S. 31-2-213. A licensed medical professional completes the disability certification, checking at least one qualifying limitation and choosing permanent (expected to last at least 12 months) or temporary (no more than six months). Disabled license plates then come from the County Treasurer.

Who completes it

The applicant completes the top section and acknowledges a possible driver license review. A licensed medical professional who has treated the applicant, or is familiar with their treatment, completes the certification with license number, signature and date; the statute refers to a licensed physician.

Submitting and payment

How to submit it
The applicant mails, faxes or emails the completed form to WYDOT Driver Services, Driver Compliance; an incomplete certification may be sent back to the medical professional.

Frequently asked questions

Who can certify a Wyoming disabled parking placard?

The form asks a licensed medical professional to certify that they have treated the applicant, or are familiar with the treatment provided, and that the condition is as stated. Wyoming statute W.S. 31-2-213 defines an eligible person as one whose walking limits are determined by a licensed physician.

Can a Wyoming placard application affect a patient's driver license?

Yes. The applicant signs an acknowledgment that issuing a disabled parking placard may lead to a driver license review, to check their ability to drive safely and that the license shows the proper restrictions for the stated condition. This applies only to people with a valid Wyoming driver license.

How long does a Wyoming temporary placard last?

A temporary placard is for a condition expected to last no more than six months, and the medical professional writes the number of months needed. A permanent placard is for a condition expected to last at least 12 months. Only one temporary placard is allowed.
