Colorado DR 2219 Disability Parking Privileges Form

Colorado's Division of Motor Vehicles uses DR 2219 to issue disability license plates and blue, purple (remuneration-exempt) or red short-term placards. A licensed professional certifies on page 5 that the applicant meets one of six walking criteria under 23 CFR 1235 and classifies the impairment as permanent, extended, temporary or short term. The seven-page form also explains renewal and self-certification rules.

Who completes it

The applicant completes page 4 with secure ID verified by a witness, and chooses plate or placard options on page 7. A physician, commissioned medical officer of the Armed Forces, USPHS or VA, advanced practice nurse, PA or podiatrist licensed in Colorado or a bordering state completes pages 5 and 6; chiropractors and physical therapists may certify short-term placards only.

Submitting and payment

How to submit it
The applicant submits the completed DR 2219 at the county motor vehicle office at the time of registration. Do not fax or email it to the Division of Motor Vehicles.
Who pays
There is no fee for placards; standard registration fees and ownership taxes apply to disability plates, and no fee is set for the professional's certification.

Good to know

  • Extended means not expected to change within 30 months; temporary means expected to last less than 30 months; short term means 90 days or less. The period runs from the date the professional enters.
  • Placards other than short term are valid for three years. Permanent impairments need professional verification initially and then every ninth year, with self-certification allowed at the third and sixth year renewals.
  • Short-term red placards last until the end of the month 90 days after issue and can be renewed only once, with a new DR 2219.
  • A mistake voids the form: do not write over, white out or cross out entries; complete a new form instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Who can sign Colorado's DR 2219?

A physician, a commissioned medical officer of the U.S. Armed Forces, Public Health Service or VA, an advanced practice nurse, a physician assistant or a podiatrist licensed in Colorado or a bordering state. Chiropractors and physical therapists may certify only for short-term (90-day) placards. The certifier gives a license number and issuing state.

What is a remuneration-exempt placard in Colorado?

A purple placard for a disability that limits fine motor control in both hands, the ability to reach 48 inches from the ground because of upper-limb weakness, or the ability to reach a parking meter because of a wheelchair or other device. The professional checks this box separately on DR 2219.

How often does Colorado require a new DR 2219 certification?

Placards must be renewed every three years. For permanent impairments, a professional certifies initially and then every ninth year, and the holder may self-certify at the third and sixth year renewals. Extended and temporary impairments need a new professional certification at every three-year renewal.
