Louisiana DPSMV 1966: Mobility Impairment Certification

Louisiana's Office of Motor Vehicles requires DPSMV 1966 for a mobility impaired hang tag or license plate under R.S. 47:463.4. A medical examiner certifies which of six qualifying conditions the applicant meets and whether the impairment is permanent or temporary. OMV rejects or returns applications that arrive without this certification.

Who completes it

The medical examiner completes the whole certification: applicant details, the condition number, permanent or temporary status, and a signature, printed name and state license number. The lower block is for the OMV analyst only.

Submitting and payment

How to submit it
The applicant brings the certification to an Office of Motor Vehicles office, together with the vehicle application (DPSMV 1799) when requesting plates.
Who pays
The form does not address payment for the medical examiner's certification.

Good to know

  • Qualifying conditions: cannot walk 200 feet without resting, needs help or a device to walk, FEV1 under one liter or PaO2 under 60 mm/Hg, portable oxygen, Class III/IV cardiac disease, or severe mobility-limiting disorders.
  • For a permanent impairment, the examiner's certification is needed only at the initial application. Temporary impairments get a one-year hang tag, and renewal needs a new certification.
  • A box marks applicants unable to appear in person at OMV; they must supply a facial photo instead.
  • Willful false certification exposes the examiner to fines or imprisonment under R.S. 47:463.4(G)(4). Each person may hold up to three additional hang tags and two plates.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Is DPSMV 1966 the same as the Louisiana physician's certification?

Yes. The OMV website calls it the Physician's Certification of Mobility Impairment, while the printed form is titled Medical Examiner's Certification of Mobility Impairment. Both names refer to DPSMV 1966, the certification required for mobility impaired hang tags and plates.

How long is a Louisiana temporary mobility impaired hang tag valid?

A temporary hang tag is valid for one year. Because improvement or recovery is expected, a new medical examiner's certification is required to renew it. Permanent impairments, by contrast, need the certification only when the applicant first applies.

Does Louisiana require a license number on DPSMV 1966?

Yes. Next to the medical examiner's signature and printed name, the form asks for a state license number, plus the examiner's address and phone. The examiner also writes the number of the qualifying condition the applicant meets.
