WV DMV-48-DI: Disabled Veteran Mobility Impaired Plate

West Virginia's DMV-48-DI is the application for a Disabled Veteran Mobility Impaired license plate. It reaches a clinician only after the veteran completes Part 1 and the West Virginia Department of Veterans Assistance certifies Part 2. A licensed physician then completes Part 3, marking the condition permanent or temporary and checking a qualifying walking limitation.

Who completes it

The veteran completes Part 1 with Social Security number, vehicle and insurance details. A veterans assistance officer certifies Part 2. A licensed physician (MD, DO, chiropractor, advanced nurse practitioner or physician assistant) completes all of Part 3 with license number and expiration date.

Submitting and payment

How to submit it
After Veterans Assistance certification, the applicant mails the form to the DMV's Mobility Impaired Plates unit or turns it in at any regional DMV office; delivery takes about 60 days.
Who pays
The veteran pays no registration fee for one plate but pays the insurance enforcement and litter fees on a 10-year schedule, plus extra for personalization.

Good to know

  • To qualify, the veteran must have a 100% total and permanent service-connected disability rating or have received a federal vehicle grant under the public laws named on the form.
  • Part 3 must be completed in full or the form is returned; physician signatures and licenses are subject to verification, and further documentation may be requested.
  • A temporary condition is valid up to six months; a permanent condition is expected to last the rest of the applicant's life.

Frequently asked questions

What must happen before a physician completes DMV-48-DI?

The veteran completes Part 1, and the West Virginia Department of Veterans Assistance certifies Part 2, confirming either a 100% total and permanent service-connected disability or a federal vehicle grant. The DMV cannot process the application until that certification is done, so the physician's Part 3 is usually the last step.

Who can complete Part 3 of DMV-48-DI?

Part 3 is to be completed only by a licensed physician, which the instructions define as an MD, DO, chiropractor, advanced nurse practitioner or physician assistant. The physician checks permanent or temporary and at least one walking limitation, such as inability to walk 200 feet without stopping or use of portable oxygen.
