West Virginia DMV-41-TR: Mobility Impaired Parking

West Virginia DMV uses DMV-41-TR for mobility impaired placards and license plates. A clinician meets it in Part II, certifying that the condition is permanent (expected to last the applicant's life) or temporary (valid up to six months) and checking at least one qualifying walking limitation. All applicants must be West Virginia residents.

Who completes it

The mobility impaired person completes Part I, including their Social Security number, and signs, or a parent or legal guardian signs for them. A licensed physician completes Part II, which the instructions define as an MD, DO, chiropractor, advanced nurse practitioner or physician assistant, with license number and expiration date.

Submitting and payment

How to submit it
The applicant mails the form to the West Virginia DMV's Mobility Impaired Placards & Plates unit in Charleston.

Good to know

  • Part II must be completed in full or the form is returned. Physician signatures and licenses are subject to verification, and DMV may ask for more documentation.
  • Criteria include inability to walk 200 feet without rest, needing an assistive device, lung disease thresholds, portable oxygen, AHA Class III or IV cardiac condition, and arthritic, neurological or orthopedic limits.
  • Replacing a lost, stolen or destroyed placard requires a new application but no new physician certification.
  • Plates go only to applicants listed on the vehicle's West Virginia registration, and permanent plates expire every one or two years.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Who can sign Part II of West Virginia's DMV-41-TR?

Part II is headed for a licensed physician, and the instructions define that to include an MD, DO, chiropractor, advanced nurse practitioner and physician assistant. The signer prints their name, medical license number and license expiration date, and checks the condition type and at least one qualifying limitation.

How long is a West Virginia temporary mobility impaired placard valid?

Up to six months. Part II defines a temporary condition as one of limited duration that is expected to improve during the applicant's life. A permanent condition is one expected to last for the rest of the applicant's life, and permanent plates then renew every one or two years.

Does the mobility impaired person have to own the vehicle?

Not for a placard: the form states the person does not have to own or drive the vehicle to use it. License plates are different and can only be issued to an applicant whose name appears on the West Virginia vehicle registration listed in Part I.
