Michigan BFS-108: Disability Parking Placard Form

Michigan residents use BFS-108 to apply for a permanent (blue) or temporary (red) disability parking placard from the Department of State. A physician, physician assistant or optometrist certifies that the person has one of the physical characteristics defined in Michigan law. The form also has a free parking section, which a clinician completes when the patient cannot easily use parking meters.

Who completes it

The applicant completes their part and signs. A physician, physician assistant or optometrist completes the medical certification, marking permanent or temporary, and completes the free parking section only if the patient qualifies for it.

Submitting and payment

How to submit it
New applications are mailed or brought to a Secretary of State office; online service handles only renewals and replacements. Temporary placard renewals need a newly certified form, mailed or brought in.
Who pays
The first placard, renewals and damaged-placard replacements are free, and lost or stolen replacements carry a fee paid by the applicant; the form does not address payment for the certification.

Good to know

  • Qualifying characteristics include blindness, inability to walk 200 feet without resting, needing a device or another person to walk, FEV1 or PaO2 lung thresholds, class 3 to 4 cardiac disease and oxygen dependence.
  • Permanent blue placards are valid four years and can be renewed online up to 45 days before expiring. Temporary red placards last one to six months.
  • Completing the free parking section on BFS-108 leads to a yellow free-parking sticker, which also requires a valid driver's license. Private lots and garages need not honor it.
  • Renewing a temporary red placard requires a licensed medical professional to complete a new application. Organizations use a separate placard application.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify Michigan form BFS-108?

Michigan law, as quoted by the Department of State, requires the disability to be determined by a physician, a physician assistant or an optometrist. Blindness may be determined by any of the three, and the other characteristics include walking, lung, cardiac and oxygen-dependence criteria.

How long is a Michigan disability parking placard valid?

Permanent blue placards are valid four years and can be renewed online up to 45 days before expiration. Temporary red placards are valid one to six months, and renewing one requires new medical certification. Organization green placards are valid four years.

How does a Michigan placard holder get free meter parking?

A placard alone does not give free parking. A licensed medical professional must complete the free parking section of BFS-108, and the person also needs a valid driver's license. The Department of State then issues a yellow free-parking sticker for the placard.
